Participants eligible for a follow-up will meet with the Community Health Worker to create a personalized care plan and will be rescreened by the Community Health Worker to monitor participant’s progress. The plan will include both a physical activity and a nutrition goal to help the participant improve both Blood Pressure and BMI. The follow-up care plan will also include helping the participant identify and set personal goals. The participant will be scheduled to meet with the Community Health Worker as recommended by the Community Health Worker depending on the individuals results of screening.
